New York State legislated the creation of the Commission on Health Care Facilities in the 21st Century in 2006 as a response to a healthcare delivery system in “crisis.” Healthcare providers throughout New York were in weak financial condition. Negative or inadequate fiscal margins limited the ability of providers to reinvest in their systems, obtain the latest technologies, access capital, upgrade their physical plants and invest in their medical staffs.

The three East End hospitals recognized this opportunity to address long standing challenges. They were faced with small primary markets, internal competition with undercapitalized facilities and key clinical services and medical staff, and a poor negotiating position with managed care plans.

Under the voluntary initiatives of the Commission, the Alliance hospitals submitted a plan to develop an integrated delivery system; right-size and regionalize services; secure emergency and acute care services at all three hospitals; and work to develop suggested affiliation agreements with Stony Brook University. The Commission adopted the plan. The East End Health Alliance was formed and awarded $14 million of available funds for implementation.
